Endeavor Care Coordinator vs. DDD Support Coordinator
Read this section before you apply anywhere, because these are two different jobs and applicants confuse them constantly.
A DDD Support Coordinator is a State of Arizona employee. They write and update the person-centered service plan with the planning team, coordinate physical health, behavioral health and long-term care services, confirm that funded services are in place, and explain members’ rights. They work for the Division of Developmental Disabilities, and you apply through the state rather than through any agency.
An Endeavor Care Coordinator works for the agency that delivers the services the state authorized. You do the staffing, scheduling, monitoring, documentation, and family communication that turns a plan into care that actually shows up.
What a Care Coordinator Does
The work splits into four areas.
Staffing and scheduling. You match Direct Support Professionals to members based on service type, location, availability, and fit, then build and maintain the weekly schedule.
Authorization tracking. Each member has approved hours for attendant care, habilitation, or respite. You want to make sure the member is receiving all the care they are authorized to receive.
Supervision and compliance. You run the supervisory and monitoring visits Arizona requires for every DSP and member pairing, keep background and registry checks current, and watch each new DSP’s training clock.
Family and team communication. You are the person a parent calls when a caregiver does not arrive, and the person who talks with the state Support Coordinator when something in the plan needs to change.
Requirements and Qualifications
Arizona does not require a nursing license or a specific degree for an agency care coordinator.
For the agency-side role, the real requirements are these. You need a Level One Fingerprint Clearance Card.
You will also clear the criminal background check and both registry checks, and you will need a valid driver’s license and reliable transportation (Endeavor offices share a company care), because in-person visits in members’ homes are a fixed part of the job.
What matters most beyond the paperwork is judgment. You are signing off on documentation that has to survive an audit, and you are the person a family reaches on a bad day.
